Kilimanjaro is a high-altitude trek, not a technical alpine climb on the standard routes. The main medical risk is altitude illness, not rock climbing. Most unsuccessful attempts happen because people go too fast, drink too little, or arrive already tired from long flights and a tight safari schedule.
Guides set a slow pace on purpose. “Pole pole” is the safety system: it gives your body time to adapt as you move from rainforest through moorland and alpine desert to the crater rim. If you feel you could walk faster, that is usually a good sign — stay with the group anyway.
Acute mountain sickness can start with headache, nausea, loss of appetite or poor sleep. More serious signs include confusion, a severe headache that will not ease, vomiting, unusual breathlessness at rest, or a wet cough. Those cases need descent, not another summit push. Diamox (acetazolamide) is sometimes prescribed by a traveller’s own doctor; Sejova Safaris does not issue medication. Discuss it at home before you fly.
Arem climbs use registered guides and porters, daily health checks, and a turnaround plan if someone is not fit to continue. Camps have limited clinic support. Serious cases may need stretcher evacuation to a lower gate and then a vehicle or, if insurance allows, air ambulance. Carry proof of cover that includes trekking to 5,895 m and helicopter evacuation — many standard travel policies exclude this.
